HIPAA Act Violation,Anthony Allen is a case manager supervisor, he violate my privacy, he communicate with people and organization that I did not authorize.Also I had to telephone DMH, on numerous times for them not to release my information!

Dmh needs an evident email! Not everyone has access to a private phone!
