Horizon Health Services, Inc. is a not-for-profit corporation that provides hope and healing for individuals and their families dealing with substance use and/or mental health disorders. Established in 1975, we are the largest provider of mental health and substance use treatment in Western New York. Horizon Health Services, Inc. is comprised of three affiliated not-for-profits: Horizon Health Services, Horizon Village and Health Management Group. Horizon Village is the corporate entity that operates our substance use disorder residential and inpatient programs. The Sanborn Campus is home to Horizon Village, Freedom Village and Delta Village. The Horizon Village Terrace House campus operates inpatient detox and crisis stabilization. Health Management Group, Ltd. (HMG) provides management services for all Horizon corporations.
